Subscription Pricing
Our automated electronic form and attendance services are designed to be simple to use and easy to afford.
The form service annual subscription fee is $280 to register up to 100 children + $1 per registered child over 100.
The annual subscription fee to add staff record forms or Child and Adult Care Food Program income eligibility forms is $100 for capacity of 100 form records + $1 for each form record over 100.
The annual subscription fee for Medication Administration Log forms is $100 for capacity of 1000 form records. Each form typically records up to 31 medication administrations.
The annual subscription fee to add short, single-use, forms such as daily health check, inquiry, injury, or maintenance check list forms is $100 for capacity of 1000 form records + $20 for additional 1000 form records capacity. Short forms typically have 25 or fewer data entry fields. After submission, single-use forms cannot be modified by the person who submits the form.
The attendance service annual subscription fee is $300 for up to 100 children + $1 per child over 100.
The annual subscription fee to add meal records to attendance is $100 for up to 100 children + $1 per child over 100.
The meal record service (without attendance) annual subscription fee is $300 for up to 100 children + $1 per child over 100.
Automated electronic forms pricing details
Form pricing is based on subscription capacity which is the maximum number of form records in the subscription account at any one time. Capacity should accommodate turn-over and occasional duplicate or blank forms submitted. Capacity can be increased at any time for an additonal annual subscription fee.
Changes to forms mandated by licensing authorities are provided at no additional charge.
Modest changes to forms requested by subscribers are provided a no additional charge. At the sole discretion of Guidestar Forms, a one-time fee may be charged for significant changes to forms.
The one-time setup fee for custom additional forms is $200. At the sole discretion of Guidestar Forms, the setup fee may be waived for simple forms.
Form setup fee is waived for one registration form per subscription and for a state standard staff or income eligibility form.
Form reports can be exported on demand at any time.
Each form subscription includes an automated weekly report for one form, usually the child registration form. An automated weekly report for an additional form is available for an additional annual subscription fee of $25.
Most centers require only our clear simple administrator instructions to start using their services. Video demonstration instructions are also available. Each subscription includes an optional 1-hour web-based training session with a live instructor.
Attendance service pricing details
Attendance pricing is based on subscription capacity which is the total number of children for whom attendance and/or meals can be recorded. Capacity should accommodate turn-over. Capacity can be increased at any time for an additional annual subscription fee.
No setup fee is charged for attendance service.
Attendance and meal record reports can be exported on demand at any time.
